I feel that Redesigning Roles, Walk the Talk, and Fine-Tuning would have been great actions to use following the Challenger disaster. Redesigning Roles could have been a way for NASA to create a cohesive bond between the different sites allowing for a more cooperative and inclusive working environment. Putting the right leadership in place that stresses the importance of all the sites being part of the NASA team instead of their own area could have allowed for more collaboration. New roles could have been implemented working on communication between the sites and initiatives for teamwork and collaboration. Walk the Talk could have been useful when they were measuring the safety of the product before launch. In the Challenger, there were several experts that told them not to launch due to the temperature and the effect it would have on the o-rings. Ultimately they asked enough until they heard what they wanted to hear and proceeded. They did much the same thing with the panels on the Columbia. If the leadership does not support the change, the others will not (Palmer et al, 2017). Fine-Tuning could have allowed NASA to look at all the safety issues as they come up and making changes as they went to try to correct flaws. Also, they could have done a better job of monitoring all the work that was contracted out to make sure that they were doing the same.
